Reviewing Care Plans

Reviewing Care Plans takes place at this time. All clients have constructed (with support of staff) their own care plan, looking at their objectives, aims and how are they going to achieve them? This is a great tool to promote clients taking responsibility and autonomy over their recovery. Our care planning programme is such that it also recognises the individuality of our clients.
