Reviewing care plans

Reviewing care plans today, a chance to reflect on the goals we have set and get support if we have not yet achieved them. We use a group therapy setting to facilitate this. clients are encouraged to not only review their care plan, but add to it if there are things that have come up since writing it, which need working on.
